- "HOLE-IN-ONE" SUBMISSION FORM -

Southern Arizona Tee Times will provide an individual who makes a Hole-in-One
a bag tag and special certificate to commemorate the achievement and recognize
them in the magazine and website @ www.sattmag.com.

The following information is required to register a Hole-In-One.
Successful Hole-In-One must be reported within 3 Days by course management or professional.
Submissions without a mailing address for recipient, will not receive a bag tag or certificate.
Email addresses are requested so they can receive SATT Digital where Hole-In-One's are also recognized.
Certificates and bag tags can take up to 8 weeks for delivery.
*** Package will be delivered to the reporting course. Course is responsible for mailing, or contacting individual for pick-up. ***
